  1. Write your name on your Research Report Assessment, FORM 7.
  2. If your teacher will also be using this form, draw lines that divide boxes in half.
  3. Score yourself on the left side of the center square boxes and on the left side of the Comments column.
  4. Read the first criteria and decide how to score your writing.
  5. Score your writing with the 1-5 Rubric scale.
  6. Turn this FORM 7 and FORM 6 faced down, under your report.
  7. They can be flipped over to be seen, but will not cover your title page and your report.
  8. Staple all pages in the upper-left corner of your Title Page.
